We reserve the right to change our privacy practices and the terms of this Notice at any time, provided such changes are permitted by applicable law, and to make new Notice provisions effective for all protected health information that we maintain. HOW WE MAY USE AND DISCLOSE HEALTH INFORMATION ABOUT YOU We may use and disclose your health information for different purposes, including treatment, payment, and health care operations. Some information, such as HIV-related information, genetic information, alcohol and/or substance use disorder treatment records, and mental health records may be entitled to special confidentiality protections under applicable state or federal law. If a person has the authority by law to make health care decisions for you, we will treat that patient representative the same way we would treat you with respect to your health information. Disaster Relief. We may use or disclose your health information to assist in disaster relief efforts. Required by Law. We may use or disclose your health information when we are required to do so by law. Public Health Activities. We may disclose your health information for public health activities, including disclosures to: Prevent or control disease, injury or disability; Report child abuse or neglect; Report reactions to medications or problems with products or devices; Notify a person of a recall, repair, or replacement of products or devices; Notify a person who may have been exposed to a disease or condition; or Notify the appropriate government authority if we believe a patient has been the victim of abuse, neglect, or domestic violence. National Security. We may disclose to military authorities the health information of Armed Forces personnel under certain circumstances. We may disclose to authorized federal officials health information required for lawful intelligence, counterintelligence, and other national security activities. We may disclose to correctional institution or law enforcement official having lawful custody the protected health information of an inmate or patient. Secretary of HHS. We will disclose your health information to the Secretary of the U.S. Department of Health and Human Services when required to investigate or determine compliance with HIPAA. Worker’s Compensation. We may disclose your PHI to the extent authorized by and to the extent necessary to comply with laws relating to worker’s compensation or other similar programs established by law. Law Enforcement. We may disclose your PHI for law enforcement purposes as permitted by HIPAA, as required by law, or in response to a subpoena or court order. Health Oversight Activities. We may disclose your PHI to an oversight agency for activities authorized by law. These oversight activities include audits, investigations, inspections, and credentialing, as necessary for licensure and for the government to monitor the health care system, government programs, and compliance with civil rights laws. Judicial and Administrative Proceedings. If you are involved in a lawsuit or a dispute, we may disclose your PHI in response to a court or administrative order. If we receive or maintain any information about you from a substance use disorder treatment program that is covered by 42 CFR Part 2 (a “Part 2 Program”) through a general consent you provide to the Part 2 Program to use and disclose the Part 2 Program record for purposes of treatment, payment or health care operations, we may use and disclose your Part 2 Program record for treatment, payment and health care operations purposes as described in this Notice. If we receive or maintain your Part 2 Program record through specific consent you provide to us or another third party, we will use and disclose your Part 2 Program record only as expressly permitted by you in your consent as provided to us. In no event will we use or disclose your Part 2 Program record, or testimony that describes the information contained in your Part 2 Program record, in any civil, criminal, administrative, or legislative proceedings by any Federal, State, or local authority, against you, unless authorized by your consent or the order of a court after it provides you notice of the court order. OTHER USES AND DISCLOSURES OF PHI Your authorization is required, with a few exceptions, for disclosure of psychotherapy notes, use or disclosure of PHI for marketing, and for the sale of PHI. We will also obtain your written authorization before using or disclosing your PHI for purposes other than those provided for in this Notice (or as otherwise permitted or required by law). You may revoke an authorization in writing at any time. Upon receipt of the written revocation, we will stop using or disclosing your PHI, except to the extent that we have already acted in reliance on the authorization. YOUR HEALTH INFORMATION RIGHTS Access. You have the right to look at or get copies of your health information, with limited exceptions. You must make the request in writing. You may obtain a form to request access by using the contact information listed at the end of this Notice. You may also request access by sending us a letter to the address at the end of this Notice. If you request information that we maintain on paper, we may provide photocopies. If you request information that we maintain electronically, you have the right to an electronic copy. We will use the form and format you request if readily producible. We will charge you a reasonable cost-based fee for the cost of supplies and labor of copying, and for postage if you want copies mailed to you. DENTAL IMPLANTS BRIDGES REMOVABLE PARTIAL / FULL DENTURES DENTAL IMPLANTS Restoration with dental implants is perhaps one of the most predictable and satisfying treatment alternatives available for patients today! Implant restorations may be used to for single or multiple teeth, or even the complete dentition. Options may consist of restorations that are cemented and/or screw-retained (fixed) or can be taken in and out of the mouth retained with various types of attachments (removable) . Providing coordinated treatment together with a dental surgical specialist and best implant dentist Manalapan NJ , Dr. Ciuffreda is able to replace non-restorable or missing teeth with state-of-the-art implant technology that has an outstanding long-term success rate. There are numerous benefits to dental implants : - Restoration of dental implants is routinely an extremely comfortable stage of treatment, almost never requiring dental anesthesia (i.e. “novocaine”). - In comparison to the significant reduction of tooth structure that would need to be removed from adjacent teeth for a bridge, neighboring teeth do not require any drilling or preparation for an implant restoration. - The surgical component of the implant contributes to the preservation of bone in the area by acting as an “artificial root”. Bone in the jaw will otherwise be lost over time without forces acting on it (through a tooth or an implant). Such loss of bone can become a cosmetic or functional concern. - Implant components consist of all non-organic components, such as titanium and porcelain. This of course means that implants are not subject to decay (getting a cavity). - Implants have an outstanding long-term success rate, with some studies reporting as high as 98%. - Implants more closely duplicate our natural biology of having individual teeth supported by a root-type structure, as compared with a bridge where two or more teeth are joined together with a connected tooth or teeth in the middle. The typical procedure for single tooth dental implant restoration: Following the extraction of the non-restorable tooth – with either local anesthesia (“nocovaine”) or under IV conscious sedation – a titanium implant (“artificial root”) will be gently and comfortably placed into the upper or lower jaw by a dental surgical specialist. Generally within 3-4 months the implant will have successfully integrated with the bone and is now ready for Dr. Ciuffreda to begin restoration. The restoration phase is almost always extremely comfortable, almost never even requires a numbing injection, nor is any drilling necessary! At the initial restorative visit , Dr. Ciuffreda will gently remove a cover screw, take some impressions, and replace the cover screw. At the final appointment, the cover screw is again comfortably removed, the implant abutment (or “post”) is placed, and a dental x-ray is taken to be sure it is positioned perfectly. The abutment (‘post”) and crown are then placed. While the abutment is always screw retained, the implant crown may either be cemented to the abutment or fabricated in conjunction with the abutment to be entirely screw retained. Call us today at 732-792-0605 TMJ DISORDERS, HEADACHE & TEETH GRINDING in Manalapan, NJ TMJ Disorders The term “TMJ disorders” (also referred to as TMD), as commonly used by patients, represents a broad array of jaw joint, occlusal, and musculoskeletal conditions of the head & neck. As the classification encompasses a wide range of conditions, there naturally are many signs and symptoms of TMD. Some of the more common ones include: – pain in front of the ear – tender or painful jaw muscles – clicking, popping or crunching noises when opening or closing the jaw – limited opening or locking of the jaw - Headaches or general head / neck pain may also be associated with TMJ disorders. (See the section on headaches below.) Such problems require comprehensive evaluation by a dentist with advanced training in this area. In evaluating a patient with symptoms of a potential TMJ disorder, our office combines the results of your medical history / questionnaire, x-rays, and a detailed examination of the head & neck to arrive at your diagnosis. Referral for imaging such as a CT scan, MRI, or iCAT may be indicated. Due to the potentially complex nature of such disorders which often involve many contributing factors, our treatment approach is one of the whole person as a system to restore harmony and balance to your TM joint / head & neck functioning. Common therapeutic approaches may include the use of an oral appliance, occlusal equlibration (adjusting the “bite”), specific jaw exercises, and the use of moist heat. We may also work with other medical specialists depending upon your individual needs, and may advise referrals for evaluating nutritional considerations, , general exercise, lifestyle, work habits and daily living changes, and stress reduction / relaxation suggestions. We have had tremendous success with our integrative, conservative, non-surgical approach to the treatment of our TMD patients. In fact, this ability to provide such profound relief from chronic debilitating pain is one of the most rewarding aspects of our practice! Headache Headache is one of the most common medical complaints of the U.S. population today and actually accounts for millions of lost workdays each year. Due to the wide range of apparent symptoms and presentations of head pain, the International Headache Society has developed a comprehensive classification in an attempt to categorize these disorders. Our experience in treating these disorders has shown that the large majority of patients who complain of unilateral headache (pain on one side) often have a muscular / myofacial (MPDS) cause of their discomfort; and a large percentage of the time are associated with an actual TMJ (actual joint involvement) disorder. The most common (although not the only) location for this type of headache is around or above one eye, travelling up from the neck on the same side. If there is an associated TMJ disorder and/or bruxism (tooth grinding), an oral appliance is usually essential to achieving relief. Such headaches of muscular origin have been quite successfully treated at our office. Coordinated referral to a physical therapist, occupational therapist, chiropractor, or a well trained massage therapist is often highly beneficial. Dr. Ciuffreda has seen numerous patients who provide a history of “migraine.” Frequently these patients have been placed on prescription migraine medication but have had only limited, if any relief. However, upon examination Dr, Ciuffreda frequently finds that they really have a muscular / myofascial type headache often associated with a TMJ disorder and/or bruxism. that can usually be alleviated without, or with minimal medications as discussed above. RED FLAGS OF HEADACHE Not all headaches are “just headaches”. There are certain signs or symptoms that may indicate a serious underlying cause for headache symptoms. If you experience any of the following it would be highly advisable for you to seek medical consultation promptly: – abrupt onset of different or new type of severe headache – “worst headache you have ever had” – new onset of headache if over 50 years old – worsening of a headache over a period of days or weeks – headache worsened or precipitated by exertion – presence of illness, fever, nausea, vomiting, stiff neck – neurological symptoms (different from classic pre-migraine signs) Teeth Grinding It has been estimated that 30% of adults suffer from tooth grinding, or bruxism. Although some people will grind & clench their teeth during the day as well, bruxism generally occurs during stage 2 of sleep.
